Pre-Visit Knowledge:
The patients need to have the pre-visit paperwork and confirmation completed before they see the doctor. The paperwork is sent to their phone number/email as well as confirmation. We are in suite D in the corner near the small palm trees. The parking lot is free and open for our patients to park in. We do not currently have a late policy or no show policy but please let the office know if you need to reschedule or are running late. Due to patient volume the office may close earlier than business hours listed, patients are more than welcome to call.
Items to Bring:
Typically, we receive all of the information needed from the pre-visit paper-work but patients should still bring their photo ID and insurance card with them. In addition, patients should bring any medical records they may already have on hand, otherwise the doctor can request other documents. The last thing they should bring is the medication bottles for any medications they are taking.
Initial Steps:
Patients will set up a new patient appointment. They can set this up either online or over the phone. They will then receive pre-visit paperwork through text/email that must be completed prior to their appointment. They will also receive a confirmation text/email to confirm if they will be at their appointment. It is crucial that the office receives this information before their appointment.
First Impressions:
On their first visit they will be greeted by the front desk staff. After that they will meet with the doctors MA. Then after that they will meet with the doctor.
Anything Else:
The most important is the pre-visit paperwork and the medical history. It needs to be filled out before the visit.
