How We Manage Hypertension

Accurate Diagnosis

We take multiple blood pressure readings at separate visits before diagnosing hypertension. We also discuss home monitoring, which gives us a more complete picture of your blood pressure patterns throughout the day.

Home Blood Pressure Monitoring

We guide patients on how to measure blood pressure accurately at home and how to log readings to share at appointments. Home monitoring helps us distinguish true hypertension from white-coat hypertension and gives us better data for treatment decisions.

Lifestyle Counseling

For many patients, particularly those with Stage 1 hypertension and low overall cardiovascular risk, lifestyle changes are the first step. We counsel on sodium reduction, the DASH dietary pattern, regular aerobic exercise, weight management, alcohol moderation, and smoking cessation.

Medication Management

When medication is needed, we prescribe and manage antihypertensive therapy, monitor your response, and adjust as needed. Common medication classes include ACE inhibitors, ARBs, calcium channel blockers, diuretics, and beta-blockers. Finding the right regimen sometimes takes adjustment.

Cardiovascular Risk Assessment

Hypertension rarely exists in isolation. We assess your overall cardiovascular risk, including cholesterol, blood sugar, weight, smoking status, and family history, and address these factors together as part of a comprehensive risk-reduction strategy.

Kidney and Organ Monitoring

Hypertension is a leading cause of chronic kidney disease. We monitor kidney function with regular laboratory testing and assess for other end-organ effects, including ordering an EKG when indicated to evaluate for cardiac changes associated with long-standing high blood pressure.

An EKG may be ordered as part of your hypertension evaluation to assess for cardiac changes such as left ventricular hypertrophy. The test is performed in the office and takes only a few minutes.

How is hypertension diagnosed?

Hypertension is diagnosed based on blood pressure readings taken on two or more separate occasions. A single elevated reading is not sufficient for a diagnosis, because blood pressure fluctuates throughout the day. We may also ask you to monitor your blood pressure at home to get a more complete picture of your readings over time.

Do I need medication if my blood pressure is only mildly elevated?

Not necessarily. For Stage 1 hypertension (130-139/80-89 mmHg), lifestyle changes alone are often the first step, particularly if your overall cardiovascular risk is low. We evaluate your full risk profile before recommending medication. If lifestyle changes are not sufficient after a reasonable trial period, medication is safe and effective.

Can I manage my hypertension with telehealth visits?

Many hypertension follow-up visits are well-suited to telehealth, particularly for medication reviews and blood pressure log check-ins. We offer virtual primary care visits for patients throughout Florida. Initial evaluations and visits requiring an EKG or lab work are done in person.

When would you refer me to a cardiologist?

We manage the majority of hypertension cases within primary care. We coordinate referrals to cardiology when blood pressure is difficult to control despite optimized medication, when there are signs of significant cardiac involvement, or when evaluation for secondary causes of hypertension is needed.
