Allergy Diagnosis & Treatment

Seasonal allergies, allergic rhinitis, sinus congestion, eczema, and asthma — diagnosed and treated by your Bradenton primary care doctor.

Allergy Care Starts With Your Primary Care Doctor

Florida has one of the longest and most intense allergy seasons in the United States. The warm climate means plants pollinate nearly year-round, and the humidity creates ideal conditions for mold and dust mites indoors. If you live in Bradenton, Manatee County, or the surrounding communities, you already know how relentless allergy symptoms can be.

The good news: you do not need to see a specialist first. Your primary care physician can diagnose and effectively treat the vast majority of allergy conditions — including seasonal allergic rhinitis, chronic sinus congestion, eczema, and mild-to-moderate asthma — often in a single visit. At Manatee Primary Care, Dr. Milian, MD, CWSP takes a thorough, evidence-based approach to allergy care, addressing both your symptoms and their underlying triggers.

When your symptoms require the next level of care — allergy skin testing, immunotherapy, or complex asthma management — we coordinate a timely referral to a trusted allergist or pulmonologist in the area.

Florida's Year-Round Allergy Season

Unlike northern states where allergy season lasts a few months, Florida's allergy season never truly ends. Here is what triggers symptoms in the Bradenton area throughout the year:

Spring (Feb – May)

  • Oak, cedar, and pine tree pollen
  • Grass pollen begins
  • Mold spores after rain

Summer (Jun – Aug)

  • Grass pollen peaks
  • Mold spores from humidity
  • Dust mites indoors

Fall (Sep – Nov)

  • Ragweed pollen
  • Mold from fallen leaves
  • Continued grass pollen

Winter (Dec – Jan)

  • Dust mites and pet dander indoors
  • Mold in humid spaces
  • Some tree pollens begin early

Conditions We Diagnose & Treat

Seasonal Allergies & Allergic Rhinitis

Allergic rhinitis — commonly called hay fever or seasonal allergies — is an immune overreaction to airborne allergens like pollen, mold, dust mites, and pet dander. It is one of the most common conditions we treat at Manatee Primary Care.

Symptoms include: sneezing, runny or stuffy nose, itchy or watery eyes, postnasal drip, scratchy throat, and fatigue. Symptoms may be seasonal (triggered by pollen) or perennial (year-round, triggered by dust mites, mold, or pet dander).

How we diagnose it: Diagnosis is based on your symptom history, physical exam, and response to treatment. When the trigger is unclear, we may order a specific IgE blood test (RAST) or refer you for skin-prick allergy testing.

Treatment options:

  • Non-sedating antihistamines (cetirizine, loratadine, fexofenadine) — first-line for mild symptoms
  • Intranasal corticosteroids (fluticasone, mometasone, budesonide) — most effective for nasal congestion and inflammation
  • Nasal antihistamine sprays (azelastine) — fast-acting for breakthrough symptoms
  • Leukotriene receptor antagonists (montelukast) — especially useful when allergies and asthma coexist
  • Allergen avoidance counseling — practical strategies to reduce exposure at home and outdoors
  • Saline nasal irrigation — reduces allergen load and soothes inflamed nasal passages

Sinus Congestion & Sinusitis

Chronic sinus congestion is one of the most frequent complaints we see in Bradenton patients, and allergies are a leading cause. When allergens inflame the nasal lining, the sinuses can become blocked, creating pressure, pain, and a breeding ground for bacterial infection.

Acute sinusitis (lasting less than 4 weeks) is usually viral and resolves on its own. Chronic sinusitis (lasting more than 12 weeks) often has an allergic or structural component that requires targeted treatment.

Treatment approach:

  • Intranasal corticosteroid sprays to reduce sinus inflammation
  • Saline irrigation to flush allergens and mucus
  • Oral decongestants (short-term) for acute congestion
  • Antibiotics only when bacterial infection is confirmed — we avoid unnecessary antibiotic use
  • Allergy management to address the root cause of recurrent sinusitis
  • Referral to ENT for structural issues (deviated septum, nasal polyps) when appropriate

Eczema (Atopic Dermatitis)

Eczema is a chronic inflammatory skin condition closely linked to allergies and asthma — the three conditions often occur together in what is called the "atopic triad." In Florida's heat and humidity, eczema flares are common and can be triggered by sweat, sunscreen, chlorine, and airborne allergens.

Symptoms include: dry, itchy, inflamed skin; red or brownish patches; small raised bumps that may weep fluid; thickened or cracked skin; and raw, swollen skin from scratching. Eczema commonly affects the hands, feet, ankles, wrists, neck, upper chest, and the bends of the elbows and knees.

Treatment options:

  • Topical corticosteroids — the cornerstone of eczema flare treatment
  • Topical calcineurin inhibitors (tacrolimus, pimecrolimus) — steroid-sparing option for sensitive areas
  • Emollient therapy — consistent moisturizer use to restore the skin barrier
  • Trigger identification and avoidance (soaps, detergents, fabrics, foods)
  • Oral antihistamines for itch relief, especially at night
  • Short courses of oral corticosteroids for severe flares
  • Referral to dermatology for biologics (dupilumab) in moderate-to-severe cases

Asthma

Asthma is a chronic lung condition in which the airways become inflamed and narrowed, making it difficult to breathe. Allergic asthma — triggered by pollen, dust mites, mold, or pet dander — is the most common form and is directly connected to the same immune pathway that drives allergic rhinitis and eczema.

Symptoms include: wheezing, shortness of breath, chest tightness, and a persistent cough — especially at night or early morning, during exercise, or after allergen exposure.

How we diagnose it: Diagnosis is based on symptom history, physical exam, and spirometry (lung function testing). We assess asthma severity — intermittent, mild persistent, moderate persistent, or severe persistent — to guide treatment.

Treatment options:

  • Short-acting bronchodilators (albuterol) — rescue inhaler for acute symptoms
  • Inhaled corticosteroids (fluticasone, budesonide) — daily controller medication to reduce airway inflammation
  • Long-acting bronchodilators (salmeterol, formoterol) — added when inhaled steroids alone are insufficient
  • Combination inhalers (fluticasone/salmeterol, budesonide/formoterol) — convenient once- or twice-daily dosing
  • Leukotriene modifiers (montelukast) — especially useful for allergic asthma
  • Asthma action plan — written plan so you know exactly what to do when symptoms worsen
  • Allergen control — treating the underlying allergy often reduces asthma frequency and severity

When We Refer to an Allergist

Primary care handles the majority of allergy cases effectively. However, there are situations where a referral to a board-certified allergist or pulmonologist is the right next step. Dr. Milian will recommend a specialist consultation when:

Symptoms are not controlled on standard therapy

If antihistamines and nasal steroids are not providing adequate relief after 4–6 weeks, further evaluation is warranted.

Allergy skin testing or specific IgE testing is needed

To precisely identify which allergens are driving your symptoms — essential before starting immunotherapy.

Allergen immunotherapy (allergy shots or drops) is appropriate

Immunotherapy can desensitize your immune system over time and is the only treatment that modifies the underlying allergic disease.

Asthma is moderate-to-severe or difficult to control

Complex asthma may require pulmonary function testing, bronchoprovocation testing, or biologic therapy (omalizumab, dupilumab, mepolizumab).

Suspected food allergy or anaphylaxis history

Food allergies and a history of anaphylaxis require specialist evaluation, oral food challenge testing, and an emergency action plan.

Occupational or contact allergy

Workplace allergens or contact dermatitis that does not respond to standard treatment may require patch testing by a dermatologist or allergist.

Our referral process: When a specialist is needed, we provide a warm referral with your full medical history and a clear clinical summary — so you are not starting from scratch at the allergist's office.

How Dr. Milian Diagnoses Allergies

A thorough diagnosis is the foundation of effective allergy treatment. At your visit, Dr. Milian will:

1

Review your symptom history

When symptoms occur, what makes them better or worse, how long you have had them, and whether they are seasonal or year-round.

2

Assess your environment

Home, workplace, and outdoor exposures — pets, carpeting, HVAC systems, landscaping, and occupational allergens.

3

Perform a physical exam

Nasal passages, throat, ears, eyes, lungs, and skin — looking for signs of allergic inflammation, wheezing, or eczema.

4

Order targeted lab work

Specific IgE blood tests (RAST) to identify allergen sensitization when the trigger is unclear or symptoms are severe.

5

Evaluate for comorbidities

Allergies frequently coexist with asthma, eczema, sinusitis, and sleep apnea — all of which affect treatment decisions.

6

Build a personalized treatment plan

A written plan covering medications, avoidance strategies, and follow-up schedule — with clear instructions for flares.
